All 8th-grade emergency medications will be sent home with the students tomorrow 6/15. If you would prefer to pick the medications up yourself, then please make arrangements with Nurse Lontai. llontai@warrentboe.org

 

Emergency medications for the 6th and 7th grade students will be sent home with them on Friday 6/17.  If you would prefer to pick these medications up instead of having them sent home with your child, please make arrangements with Nurse Lontai.

2022 WMS Fall Athletic Information

 

Important Dates: 

 

August 4th: Genesis Registration opens for FALL athletics

 

September 1st: Deadline for the submission of  FAll Sports Physicals/Paperwork

 

September 7th: First day of tryouts/practice: Boys and Girls Soccer

 

September 12th: First day of tryouts/practice for Cross Country

All prospective athletes must have a current sports physical on file in the health office. Physical examinations are good for 365 days from the date of exam.  If a physical has been completed more than 90 days prior to the first practice, a Health History Update questionnaire must be completed.

 

Completed forms can be uploaded into the Genesis portal, or they can be dropped off or mailed to Warren Middle School 100 Old Stirling Road Warren NJ 07059.

 

Please note:  Sports physical forms are not cleared on the same day that you drop them off at the Health Office.  Sports physical forms get reviewed & cleared by our school physician.  A parent/guardian will receive a sports clearance email notification once approved by our school physician
