Physical Examinations
A physical examination is required for each new student who enters the school district. A New Jersey licensed physician or nurse practitioner must do the physical.
While not required, it is important to obtain a physical examination at least once during each of the developmental stages: early childhood (K-3rd); pre-adolescence (4-6 gr); adolescence (7-12 gr).
Please use the Warren Township Physical Exam form found on the Health Office Website. Please DO NOT use the Universal Child Health Record form from your pediatrician's office.
