The following state mandated forms must be completed if you have never tried out for a sport or your sports physical on file is more than one year of the first day of practice/tryouts.

• Health History (English | Español)
• Annual Athletic Pre-Participation Physical Examination Form
• PPE Medical Eligibility Form
• Athletes with Disabilities Form: Supplement to the Athlete History

The Health History Update Questionnaire must be submitted for each season of participation if your complete physical is dated more than 90 days from the start of the season. (Hard copies will no longer be accepted). You will also need to complete the forms above if your physical was completed more than one year of tryouts/first day of practice of your sport.
