6th grade Immunization Requirements

 

The required vaccines to enter 6th grade are:

Tdap (Tetanus, diptheria, and pertussis)

Meningococcal Vaccine

 

N.J.A.C. 8:57-4:10 and N.J.A.C. 8:57-4:20 requires that children born on or after January 1, 1997 and enrollled in grade six or transferring into a New Jersey school from another state or country to receive one dose of meningococcal vaccine and one booster dose of the Tetanus, diptheria and pertussis (Tdap) vaccine.
